Questions not appearing

Iam trying to develop a survey.I created a dummy survey and used php to copy all the questions into new surveys.
The problem i have encountered is it is not copying 2 questions in all surveys.It just leaves the number of the question
In dummy survey i have 1,2,3,4,5,6,7,8,9,10,11,12,13,14.
where as in new survey
I have questions 1,2,3,4,5,7,8,9,10,12,13,14.
It just discards 6 and 11 th questions in all surveys.
What could be the possible reason.
Can some one help me regarding this.
